Update: The company has stated that only 74% of it's members have completed the dependent verification. This leaves about 35,000 employees that have not been verified. If you have not taken action you must by close of business on June 15, 2012.

Verizon will be conducting an audit of all covered participants in their benefit plans beginning April 17, 2012. You MUST respond.

If you do not respond, unverified dependents will be removed from coverage effective August 1, 2012. You will receive a package at your home that will explain the process in full. Please see the attachments below for more details on the documents and materials that need to be provided.

• 4/17 - Packages mailed to home address on file.
• 5/15 - Reminder mailed.
• 6/15 - Communicated end date.
• 6/20 - Coverage termination notices sent.
• 7/13 - Grace period ends.
• 7/31 - Termination of benefits for unverified dependents.
